Policies & Practice Guidelines
Last updated: 2025-11-24
Patient Information Requirements
To remain in compliance with state medical regulations, all patients must maintain an accurate and complete Patient Profile. This includes:
- Full legal first and last name
- Date of birth
- Phone number
- Email address
Each patient is responsible for managing their own appointments and medical records. We do not allow booking on behalf of friends or family members. Appointments are non-transferable.
Booking an Appointment
A valid credit card on file is required for every individual appointment at the time of scheduling. This secures your appointment time with your provider.
Appointment Confirmations
Confirmation reminders are sent via text and email 3 days prior and 24–26 hours prior to your appointment.
It is the patient’s responsibility to monitor, confirm, or modify their appointment in a timely manner. Late replies may not be accepted and may result in applicable fees.
Rescheduling & Cancellation Policy
All appointments must be rescheduled at least 24 hours in advance.
To avoid a $75 cancellation fee, appointments must be rescheduled — not cancelled — within the 24-hour window.
Missed appointments (no-shows) will result in a $75 fee.
A maximum of 3 reschedules is permitted for any single appointment. Additional changes may incur fees.
For fastest service, please use the Patient Portal to reschedule or cancel your appointment. You may also text us or email our support team.
Late Arrivals
Please plan to arrive on time for your appointment. A 10-minute grace period is permitted.
Arrivals beyond this timeframe are considered a NO SHOW, and a $75 non-refundable fee will be charged.
If you arrive late and the provider is able to accommodate you, please note that services may be limited due to time constraints, and the late/no-show fee will still apply.
Neurotoxin Touch-Up Policy
Initial Results
Neurotoxin results develop gradually and may take up to 14 days to reach full effect.
Touch-Up Eligibility
Your treatment includes one complementary touch-up, if needed, to fine-tune results. Touch-ups are scheduled based on provider recommendation, typically 2–3 weeks after your initial treatment.
Scheduling Requirement
Touch-up appointments must be scheduled on the same day as your initial treatment to ensure availability within the proper time window.
No Touch-Ups After 3 Weeks
Due to the nature of neurotoxins, touch-ups cannot be scheduled or performed after 3 weeks from the initial appointment.
Communication & Response Times
Our in-office patients are always our top priority. If we are unable to answer your call, please leave a voicemail so our team can return your message.
Emergency or time-sensitive messages are addressed first.
For general inquiries, please allow 1 full business day (Mon–Fri, 9:00 AM – 4:30 PM) for a response.
Your Patient Portal is available 24/7 to:
- View, cancel, or reschedule appointments
- Book new services
- Complete required forms
- Sign consents
- Update your information
